CCAP is your family-centered medical and dental care provider in Cranston, Warwick and Coventry.

The Comprehensive Community Action Program provides Rhode Island communities with quality medical, behavioral and dental services they can afford. At CCAP, we not only help you achieve and maintain your overall health, but we also offer behavioral health services and dental care. Our board-certified doctors, nurse practitioners, dentists, hygienists, licensed clinical social workers and other health care professionals are knowledgeable, caring and understanding. They are dedicated to getting you healthy and keeping you healthy.

We understand that despite the Affordable Care Act, nearly 28 million Americans remain uninsured, and many of those that do have coverage are under insured. We accept all third party insurances including Medicaid, Medicare and RIte Care. Self-pay and sliding scale fee are available for those who do not have health insurance and meet financial eligibility requirements.

Our experienced staff includes physicians, nurse practitioners and other healthcare professionals who are caring, compassionate and highly skilled. We won't turn you away, even if you can't pay. Family Medicine

Your primary care team also provides care management services to ensure your health care is comprehensive, coordinated and beneficial to your quality of life. If you have diabetes or cardiovascular disease, we provide vital information and clinical tracking that help you achieve more positive outcomes.

CCAP Dental Services for Adults and Children

Our oral health professionals, located in Family Health Services of Cranston, Everett C. Wilcox Health Center of Warwick and coming soon to our Family Health Services of Coventry, provide dental services to children, adolescents and adults. Whether you have an issue requiring immediate care or simply need to schedule a cleaning, our dental teams are available to help.

We also offer two special options for children. The Molar Express is our mobile unit that provides pediatric dental care, and West Bay Smiles is a school-based dental care program for patients aged two to 21 in Cranston and Warwick.

Food and Nutrition

  • Women, Infants & Children (WIC) Supplemental Nutriritional Program - a statewide program for pregnant and parenting women with infants and children under the age of 5 years.
  • Monthly food vouchers to purchase nutritious food

We accept self-pay patients, all third party insurances including Medicaid, Medicare and RIte Care. We offer patients who qualify and don't have insurance or don't have enough insurance a sliding fee discount program.

CCAP Sliding Fee Discount – We offer a sliding fee discount for individuals who meet financial eligibility requirements. We do not deny care due to inability to pay.

CCAP is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n). This health center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
