CCAP's Family Health Services Diabetes Program Wins Award

CCAP Family Health Services of Coventry received the 2007 Best Sustaining Team Award for their work in the Diabetes Collaborative.

This award was presented during the Rhode Island Chronic Care Collaboratives Outcome Congress held on June 27 at the Crowne Plaza in Warwick. The award was presented by Neighborhood Health of Rhode Island.

There are 31 teams in the RICCC. Coventry received the award for the best team in the 31. Family Health Services of Cranston received an Honorable Mention.

CCAP's Family Health Coventry and Cranston have been participating in the Chronic Care Collaborative since the fall of 2004.

Recognized core measures for Diabetes Care are tracked through a computerized data entry system. Data entry enrolls a patient into the tracking system. Reports of core measures are run and reported on a monthly basis.

The diabetes team meets monthly to assess data and recommend methods for improving care to diabetic patients. Some of our methods implemented to date are: a revised diabetic progress note, a color coded prompt flag system to prompt providers of needed core measures, reports of patients not seen in six months, reports of patients needing foot checks, and MA's removing socks and shoes at each visit.

Coventry has surpassed recommended goals in self- management, blood pressure less than 130/80, cardiac risk reduction - ACE and ARB medications.

They have improved in all other areas. This is definitely a team effort. The diabetes team prompts for recommended core measures and runs reports. However, the health care of the patient is provider driven and reflects the excellence of care patients receive at CCAP Family Health Services. All other staff including nurses, MA's, desk staff, medical records, referrals, and recall staff are key to achieving such excellent results.
